Document Retrieval Request SystemTo retrieve payment voucher supporting documentation filed by the Office of Administration, Division of Accounting, the completion and submission of a Document Retrieval Request Form is required. Instructions and form are below. Document Retrieval Request Form Instructions - Follow the instructions to complete the Document Retrieval Request Form. Document Retrieval Request Form - Opens in a window that allows you to resize the window for side-by-side or top-to-bottom viewing. This is helpful when entering the 6-digit State Record Center Box Number from the below Catalog of Documents listing. Catalog of Documents - From the list below, select the appropriate fiscal year (the State fiscal year begins July 1 and ends June 30 of the following year) and month to complete the State Record Center Box Number field on the Document Retrieval Request Form. If you do not have Microsoft Word available to view the Catalog of Documents, then download MS Word 2003 Viewer and follow instructions for its installation. If you do not have Adobe Acrobat Reader, download the Reader and follow instructions for its installation.
