To avoid the delay of having a paper copy mailed to you, you may want to do one of the two following options.

1. Open the application as a PDF document. You will need to print out the application, then complete it using a pen or typewriter. If needed, you can download the free Adobe Acrobat Reader by clicking the icon at left.

2. Download and install the application on your computer. You will be able to enter your information into the form on your computer, print it out and sign it.
