This is senior-level professional and supervisory work on an inter-disciplinary case management and/or assessment team in a comprehensive purchase-of-service and community placement program for persons with developmental disabilities.

Minimum Qualifications

Applicants must meet one of the following eligibility requirements:

(More specific qualifications may be required for particular positions allocated to this class depending on the assigned duties and responsibilities.)

A Bachelor’s or higher level degree from an accredited college or university or a Registered Nurse License; and,

Three or more years of professional experience as a member of an inter-disciplinary team assessing, developing, or implementing Individual Support Plans for persons with developmental disabilities.

 

 

Job Duties

This description may not include all of the duties, knowledge, skills, or abilities associated with this classification.

Plans, coordinates, and supervises the work of an inter-disciplinary case management and/or assessment team; assigns cases and evaluates the work of the case management and/or assessment team members.

Monitors the development and implementation of support plans or the assessment process; reviews and approves Individual Support Plans or assesses support needs.

Develops, modifies, and assesses programs for meeting long and short-term goals for clients.

Evaluates services that are purchased and provided by Case Managers.

Works closely with an administrative supervisor in planning, assessing, and determining program needs for all clients residing within the assigned geographic area.

Conducts special studies and makes recommendations concerning the development and implementation of programs for persons with developmental disabilities.

Participates with vendors in developing new programs, assessing programs, or modifying programs to upgrade the quality of services.

Conducts group and individual training sessions for case management and/or assessment team members.

Manages a selected caseload of clients with developmental disabilities in placement or assessment.

Makes presentations concerning program objectives and services for persons with developmental disabilities.

Prepares periodic reports and maintains accurate records.

Exercises independent judgment and initiative in planning and coordinating client services; receives general administrative supervision.

Performs other related work as assigned.

Key Skills

Comprehensive knowledge of case management methods, principles, and techniques in regard to support or assessment services.

Comprehensive knowledge of various developmental disabilities and corresponding vendors and services available for clients.

Comprehensive knowledge of statutes, administrative rules, and regulations relating to program operation.

Comprehensive knowledge of the behavioral sciences and allied disciplines involved in the evaluation, care, and training of persons with developmental disabilities.

Ability to plan, coordinate, supervise, and evaluate the work of an inter-disciplinary support or assessment team.

Ability to develop, coordinate, and evaluate services within an assigned geographic area.

Ability to supervise the development of long and short-term goals for clients.

Ability to establish and maintain effective working relationships with staff, community agencies, family members of persons with developmental disabilities, and the general public.

Ability to communicate effectively.
