A Facility Request is an avenue for the requesting agency to let the Division of Facilities Management, Design and Construction know there is a change in space requirements at a State-Owned or Leased building. A completed form must be submitted any time an office size, location or configuration requires modification. The requesting agency will provide detailed information which will assist in determining the need for change in current allocated space.
All requests must be submitted on the Facility Request Form.
After the request has been reviewed and approved by OA Facilities Management, Design and Construction a Space Analysis will be requested that identifies in detail the specific office requirements. A detailed space analysis is not required until the initial FACILITY REQUEST has been reviewed and approved by OA Facilities Management, Design and Construction.
