All state employees and contractors with an office or workspace located in a State-owned or leased building maintained by the Office of Administration, Division of Facilities Management, Design and Construction (FMDC) are required to have a State ID Badge.

Authorized Representatives shall submit all requests for a Badge using the ID Badge/Building Access Application form. This application must also be used to request access to building entries during non-public times (See State-Owned Building Hours of Operation) or secured interior doors within State buildings.

Yes. FMDC Badge Office must be notified when a badge is no longer needed. All badges must be returned to the appropriate Badge Office Processing Location.

When a badge needs to be deactivated due to being lost, stolen, or end of employment, an application is not required.

No. An individual cannot carry multiple state issued badges.

Photographs may be submitted to the badge office electronically via email attachment or USB. Completed badge applications and photographs must be submitted before processing can be completed. If the photographs are not compatible with the ID badge system, FMDC will notify the Authorized Representative that a different photo is required or that an appointment must be set up with FMDC to have the photograph taken at the appropriate processing location. A completed ID Badge/Building Access Application and an appropriate photo (State ID Photo Basics and Requirements) should be submitted by the Authorized Representative.
Can I send an employee to a processing location for an ID badge if they have all of the required documents?

To avoid extended waits and delays for employees, FMDC asks that requests for State ID Badges be submitted prior to sending the employee to the processing location. A scheduled appointment is preferred.

State ID Badges are valid for 6 years. Temporary badges such as contractors, interns, and seasonal staff badges are valid for 1 year. FMDC Badge Office will send a quarterly report to the Authorized Representative prior to expiration to complete the badge renewal process.
